AirTrain connects terminals to Newark Airport rail station ($5.50 one-way, paid at the station). NJ Transit trains then run to New York Penn Station (25–30 min, $13.50) or Newark Penn Station (5 min). Total to Midtown Manhattan: $19 and about 45 minutes.
